Information for patient participation groups and GPs

If you are a patient group or a GP surgery or other health care group wishing to display information about Share My Care, you can download materials here.

Who is involved in Share My Care?

The programme is being led by Virgin Care, who provide adult and children’s community health and social care within BaNES. We are working with other healthcare providers to allow us to look after the record of the xxxxx people who live in the area.
