Supported Living Service

The service supports adults with a Learning Disability and Autism to live independently within their own homes or tenancy. This can include support around medication, personal care, budgeting, life skills, promoting independence and being part of community.

The service has an enabling ethos where service users and empowered to be involved in all aspects of daily living. We work within the city of Bath and surrounding area.

To receive a service you must entitled to a Personal Budget. Referrals are received from Case Managers from the Social Care Team who have a learning disability or autism diagnosis. The support is then provided to meet an individual’s eligible needs and determined by a Care Act assessment.

The service supports a number of service users, most fairly independent, and only requiring minimal support with maintaining their tenancy and daily living skills in order to live independently in the community however we can offer 24  hour care should your needs require this.

We are outcome focused and the support we provide in aligned with your assessed eligible needs as well as being goal focused to develop in independence and skills.

The service is registered with CQC (Care Quality Commission) for personal care.
