Who is this service for?

Any adult of 18 years and over who has been diagnosed with an Autistic Spectrum Condition and who is funded by Bath and North East Somerset.

Or

A young person (from 16 years) during their transition from Children’s to Adult Services who has been diagnosed with an Autistic Spectrum condition.

We offer support with:

  • Completing an assessment of your health and social care needs
  • Reviewing existing service and support
  • Completing Carers Assessments and providing advice to carers
  • Providing advice and guidance on issues around benefits, housing, employment, education and training
  • Providing information and signposting to appropriate services
  • Setting up personal budgets and assisting you to find an appropriate service

The support we offer will include directly supporting service users, their families and carers through crisis and facilitating change which leads to improved wellbeing, life skills, independence and opportunities. The offer of a Care Act Assessment is usually our first point of contact with service users. This will identify someone’s eligible care and support needs. The team then carries out the support planning and micro-commissioning required in order to meet these needs including the setting up of direct payments.

We have strong links with the Complex Health Needs Service and other local providers who work within our locality. We signpost and make direct referrals to local services. This holistic approach is vital to achieve good outcomes for service users and manage complex decisions and risk.

We will also work with you to keep you safe, promote independence and facilitate change. This could be support in getting a job, moving home or learning new skills. This is likely to include working closely with other agencies to meet these outcomes.

How do I access this service?

You can refer yourself to the service or a professional involved in your care can refer you.

We are only able to provide case management support and therefore we are unable to provide diagnoses.

If you would like to be considered for a diagnosis you would need to obtain agreement from your GP to access BASS @ Bath and North East Somerset Diagnostic Service. Please email vcl.bathnesbassin@nhs.net for more info.

What happens next?

Once you have been referred to the service, a letter will be sent to you acknowledging receipt of the referral.

A member of the team will later contact you to arrange an appointment to complete a Community Care Assessment of your needs.

What does this service cost?

If you are eligible for social care services under the Care Act 2014 you will need a financial assessment and may have to make a financial contribution towards the cost of your care (this would be in line with Bath and North East Somerset council’s financial assessment policy).
