The Community Neuro and Stroke Service aims to help people regain skills and become more independent by providing rehabilitation and management in their home and local community.

This specialist service is for adults over the age of 18 and addresses the needs of service users who have had a stroke and/or have long-term neurological conditions requiring community support (this could include movement disorders, Multiple Sclerosis, Motor Neurone disease).

The service consists of a number of different professions, including physiotherapists, speech and language therapists, occupational therapists, clinical psychologists, therapy assistants, specialist nurses, and a consultant in rehabilitation medicine. Together they provide intensive early supported discharge, planned rehabilitation and management throughout the care pathway.

The service includes the following teams:

Early Supported Discharge for Stroke

Planned Rehabilitation (Neuro Team)

This service can be accessed by a referral from your GP, Consultant or health care professional.
