Adult Social Care

Our service consists of Social Workers and Care Service Co-ordinators who undertake needs assessments and help you to plan the support you need, as well as supporting carers in their caring role.

We support any adult over the age of 18 who has care and support needs, and who lives in the Bath & North East Somerset area.

Our team of Care Advisors, Social Workers and Care Service Co-ordinators will discuss your needs with you and offer you information and advice. If appropriate, we will offer you an assessment and help you to plan the support you need. We also offer assessments for carers.

Our aim is to promote your wellbeing and to help you live independently for as long as possible.  We will enable you to access the support that you need, this may include referring you to our Reablement Service to help you maintain your independence, referring to a domiciliary care service as part of your care and support plan, signposting you to local support groups or helping you to set up a direct payment to purchase your own care and support.

How do I access this service?

You can refer yourself to this service, or you can be referred by your GP or other health care professional. Please use the contact details for First Response Team(Advice, Signposting, Information and Safeguarding Team) on the right of this page to self-refer or request information.

If you are concerned about a family member, friend or neighbour, you can refer them to First Response Team who can respond to any new Safeguarding Adults concerns as well as dealing with all initial social care enquiries and referrals.

What does this service cost?

If you are found to require care and support following a needs assessment, Bath and North East Somerset Council will carry out a financial assessment of your circumstances to identify how much you may be required to contribute towards the cost of your care.
