X-PERT programme for people with type 2 diabetes
X-PERT is a six-week group education programme for patients with Type 2 diabetes. It has been developed to help patients understand more about diabetes and how they can manage it through lifestyle changes. It is suitable for anyone newly diagnosed and those who have had diabetes for a while but want to learn more.
The X-PERT Programme is based upon the theories of patient empowerment, discovery learning and patient-centred care. Its main aim is to develop patients’ knowledge, skills and confidence to enable them to make informed decisions regarding lifestyle and diabetes management.
Each programme will be run by an trained and experienced health professional who will guide patients through all aspects of their diabetes. Patients attend a 2 ½ hour session each week for 6 weeks. Each week a different topic will be covered and at the end of each week a goal will be set.
It is important where possible that patients attend all the sessions to fully benefit from the course.
The X-PERT course will run on different days and times of the week in various venues around Bath and North East Somerset. X-PERT courses are planned to commence from November.
