The Active Ageing service is provided by Health Visitors. The team aim to enable older people to continue to be independent in their own homes and to promote quality of life by improving health and reducing inequalities, with a focus on social, emotional and physical health.
This service is for adults aged 80-84 years old who are registered with a GP in Bath & North East Somerset.
If you are over 84 and would like to meet us, please ask your GP for a referral.

The team consists of Health Visitors and Health Visitor Support Workers who are based in the community. We are able to help people access medical, social, home environment and financial services. We also support carers.
The Active Ageing Team works in partnership to help identify health needs and to signpost families to the services they may require in order to help maintain independence.
The service offers:
- Falls and accident prevention
- Advice on memory loss and dementia
- Support for those who have been recently bereaved
- Advice on managing long term health conditions
- Health education
- Signposting to local and national support services

You can refer yourself to this service by calling the number provided at the top of this page to find out who your local Health Visitor is and how to contact them. Once contacted, they will then offer you a home visit if appropriate.
You can also get in touch with your GP or other healthcare professional, who will refer you to your local Health Visitor.
